A deposit is required when scheduling an Initial Consultation. This deposit is applied toward the cost of your visit. The

For certain laboratory tests, fasting may be required. This means no food or beverages, other than water, after midnight the night before your labs. You may drink water and take essential medications unless your provider instructs otherwise. These laboratory instructions may apply to both men and women depending on the tests ordered.
For patients who menstruate, some hormone testing must be completed during a specific window of the menstrual cycle for accurate interpretation. When scheduling your labs, please ensure they are completed between days 19 through 21 of your menstrual cycle unless your provider gives you different instructions.

Prior to your appointment, you will be asked to complete intake forms and provide relevant health history information.
In some cases, laboratory testing may be recommended prior to your consultation so your provider can review your
results in advance. We ask that your intake forms be completed at least 72 hours prior to your appointment and your
labs to be completed one week prior.
After scheduling, you will receive instructions regarding any required forms, laboratory testing, or preparation needed
for your visit.
Hormone therapy consultations focus on evaluating symptoms that may be related to hormonal imbalances. During
this visit, the provider reviews your medical history, symptoms, and laboratory testing to determine whether hormone
optimization may be appropriate. Hormone therapy is commonly used to address symptoms such as fatigue, mood
changes, low libido, sleep disturbances, or metabolic changes related to declining hormone levels in both men and
women.
Functional nutrition consultations focus on identifying underlying factors that may be affecting overall health,
including digestion, energy levels, inflammation, metabolic health, and hormone balance. This visit takes a root-cause
approach and may include detailed evaluation of diet, lifestyle, and nutritional status. Personalized nutrition strategies,
lifestyle recommendations, and targeted supplementation may be discussed to support long-term health and wellness.
Medical weight loss consultations focus specifically on weight management and metabolic health. During this visit,
your provider evaluates your health history, weight-related concerns, and goals. A personalized plan may include
nutrition guidance, lifestyle recommendations, and when appropriate, medically supervised weight loss treatments
designed to support safe and sustainable weight loss.
In some cases, these services may overlap, and your provider may recommend a combination of approaches
depending on your health goals. If you are unsure which consultation type is most appropriate, the practice can help
guide you toward the visit that best fits your needs.
Our practice does not offer hormone pellet therapy. Instead, we focus on treatment options that allow for more precise
dose adjustments and ongoing monitoring. Your provider will discuss the treatment options available and determine
what approach is most appropriate for your care.
Off Coast Wellness operates on a self-pay model. The cost of care may vary depending on the services provided,
laboratory testing, and any recommended treatment plan. Prior to beginning care, patients will receive clear
information regarding visit fees and potential costs so they can make informed decisions about their care.

Eligibility for semaglutide or tirzepatide is typically determined based on factors such as your body mass index (BMI), overall health, and weight loss goals. These medications are generally prescribed for individuals with a BMI of 30 or higher, or for those with a BMI of 27 or higher who have weight-related health conditions like type 2 diabetes, hypertension, or high cholesterol. During your consultation at Off Coast Wellness, we’ll assess your health and determine if semaglutide or tirzepatide is an appropriate option for you.
Hormone therapy may be appropriate for some patients depending on their symptoms, health history, and laboratory
results. At Off Coast Wellness, treatment decisions are made following an individualized consultation with a licensed
healthcare provider. During your appointment, your provider will review your medical history, discuss potential
benefits and risks, and determine whether hormone therapy may be appropriate for you. Treatment plans are
personalized and based on each patient’s unique health needs
Hormone therapy outcomes can vary depending on factors such as dosing, monitoring, lifestyle considerations, and
overall health. Your provider will review your previous treatment history, laboratory results, and symptoms to
determine whether additional evaluation or a different approach may be appropriate.
Off Coast Wellness does not provide hormone therapy or related treatment for patients who are currently pregnant or
actively trying to become pregnant. If you are pregnant, planning pregnancy, or undergoing fertility treatment, we
recommend seeking care from an obstetrician, reproductive endocrinologist, or other appropriate specialist.
Appointments are designed to allow adequate time for providers to review your health history, symptoms, and
laboratory results in detail. Because of this approach, appointment availability may sometimes be limited. The online
booking system on our website always reflects the most current appointment availability. If you are looking for the
soonest available appointment, we recommend checking the online booking page, as it shows the latest openings in
real time
If appointment availability is limited, patients may request to be added to the waitlist to be notified if earlier
appointments become available. Availability may change as schedules adjust. If you would like to be added to the

Off Coast Wellness offers both in-person and telehealth services depending on the type of appointment and applicable
licensure requirements. Hormone Optimization and Medical Weight-loss services are available in person in Delaware
and available via telehealth in Delaware, Maryland, and Florida. Functional Nutrition services are available in person in
Delaware and via telehealth in Delaware and Maryland.
Telehealth appointments allow patients to meet with their Off Coast Wellness provider remotely using secure
technology. During a telehealth visit, your provider will review your symptoms, health history, and laboratory results
and discuss potential treatment options when appropriate. In some cases, additional laboratory testing or follow-up
care may be required before or after your appointment. Telehealth services are not appropriate for medical
emergencies.
Patients who have established care with Off Coast Wellness communicate with the practice through our secure
messaging platform. Information about secure messaging and communication will be provided during the onboarding
process.
